Birth Preferences Form (Editable)
This fully editable template is great to give clients before birth! For midwifery clients, it provides the cliff notes on various birth preferences that may not already be part of default practice. (You won’t see things like delayed clamping, etc listed because that should be standard practice for midwives!)
As a BONUS you’ll also get access to a Birth Preferences Form specifically for hospital transfers! This takes into account some of the options that may still be available to families when experiencing a change in birth plans unexpectedly. This can prove helpful to staff as it provides the cliff notes of what the family prioritizes most.
(You won’t see things like “no monitoring” listed as planning a hospital birth is very different from transferring mid-labor. If we are transferring from the OOH space, that means our risk status has changed and we’ll be needing some of those interventions the hospital has to offer.
This client handout is ready to print as-is or you can edit in Canva and add your own pictures, fonts, colors, info and more!
